Health Navigators Help Dallas County Residents Thrive Amid Increasing Need 2024

Dallas County Iowa LogoDate: Sept. 10, 2024. Adel, IA – Dallas County Health Navigation, a program of the Dallas County Health Department, has released the FY24 Annual Report outlining the program’s efforts to connect Dallas County residents with the basic resources needed to thrive in our communities.

Over the last year, Health Navigators directly served 580 Dallas County families.

The Health Navigation program was launched in 2010 to address barriers people were facing while trying to access healthcare. It has since grown to address all the social determinants of health, connecting residents with resources for over 1500 needs this last year. “We talked about social problems, health problems, and employment problems” stated a former Health Navigation client.

In FY24, Health Navigators visited with families experiencing more needs than past years and saw more difficulty connecting Dallas County residents with resources they needed. “Many of our community partners are strained by the increasing need in our county. Our navigators have done a great job helping our clients to problem solve when the resource we would normally connect them with is out of money or at capacity.” says Abigail Chihak, Community Health Administrator and manager of the Health Navigation program. Despite increasing demands, the Health Navigation program maintained a 89% resource connection success rate.

Dallas County Health Department employs three full time Health Navigators. Health Navigation visits are completed at the client’s home, or wherever they feel most comfortable, at no expense to the client. The only eligibility requirement is Dallas County residence. To connect with a navigator, call 515-993-3750 or complete our online referral form.

The Dallas County Health Department will present the findings of the FY24 Annual Report at 9:00AM during the Dallas County Community Health Coalition, Thursday September 12th located at the Dallas County Human Services Campus. All are welcome to attend.
